This really only applies to the Patriarch of Elinie, although a highly ranked Paladin within a temple could be called a Patriarch as well.

Err, if anything the Patriarch title is one of the more common in Anuire. Same goes for various mutations, such as High-Prefect, Auxiliary-Prefect, and probably Sub-Prefect as well. (Although it is obviously 'High-Prefect' is just some shameless self promotion ;D )

Unlike the noble titles which is pretty much 'agreed' throughout Anuire, the individual heads of the various temples probably strongly disagree which title is the ranking on.

Well, as I said, this *is* from another Pbem, who have different titles for things, just as we have viscount and such here. It would need tweaking to use.
It just seems odd with everyone going around refering to peoples titles or first names all the time, when you would probably adress them as "Your Excellency" instead. You don't see people walking up to the Pope and saying "Hello, Pope, how are you?" (you generally don't see people walking up to the Pope like that....)
